Wheatland High School Volleyball Fundraiser

(proceeds will help pay for WHS team camp)

Bulldog Summer Camp 

Date: July 12, 13, and 14  Cost: 3rd -5th $30;    6th-12th-$40

*Price includes a t-shirt, we prefer registration by July 1st.

Camp will be held at Wheatland High School.

I certify that my child has medical insurance.  I grant permission for my child to participate in the clinic and I hereby assume all risks of her personal injury that may result from clinic activity.  As parent/guardian, I hereby release Sally Nichols and Platte County School District #1  from all liabilities, including claims and suits at law for injury which may result from my child taking part in the clinic.  I agree to assume all costs of treatment.  I understand I will be responsible for any medical or other charges in connection with attendance at this clinic.
